Service Eligibility

In order to qualify for PCA services, you must be eligible and enrolled in a Minnesota Health Care Program that includes PCA Services. You must also have an assessment by a lead agency public health nurse or certified assessor to determine what services you qualify to receive.

All services are paid for through health care plans, government waiver programs or private pay, and must be preauthorized by the designated payer.

Nurse Supervision

ReliaCare PCA services are provided under the supervision of an experienced Registered Nurse in accordance with Minnesota State guidelines and regulations.

PCA Certification

ReliaCare is certified by the Minnesota Department of Human Services (DHS) to provide PCA services. All ReliaCare PCAs are trained, certified and registered with DHS as personal care assistants.

Family Care Services

If you are using a government waiver to pay for services, you have the option of using a qualified family member to provide your personal assistance care. We’ll gladly provide the required training to your family member and guide you through the process of getting your family member registered as your PCA.
